They must be supervised and performed in a cosmetic medical facility, and only a facility with a physician or nurse practitioner on site may advertise as a "medical spa."
What counts as a cosmetic medical procedure
- Utah Code 58-67-102 defines a cosmetic medical procedure to include using cosmetic medical devices for ablative or nonablative procedures and injecting a medication or substance, including a neurotoxin or a filler, for cosmetic purposes. The practice of medicine includes performing cosmetic medical procedures.
- Ablative procedures are expected to excise, vaporize or remove living tissue, including carbon dioxide and erbium:YAG lasers. Hair removal counts as a nonablative procedure.

Supervision and delegation
- Anyone authorized to perform a cosmetic medical procedure must be supervised by a physician or APRN with an unrestricted Utah license, and cosmetic medical procedures may only be performed in a cosmetic medical facility (Utah Code 58-1-505).
- For nonablative procedures other than hair removal, a physician, nurse practitioner or physician assistant must develop a treatment plan and evaluate the patient in person or by live telemedicine before treatment starts. Registered nurses and master estheticians work under general supervision, with the supervisor available in person and within 60 minutes or 60 miles; estheticians and practical nurses need direct supervision, with the supervisor present.
- For laser hair removal, medical assistants may also perform procedures under indirect supervision, with the supervisor in the facility. Anyone delegated a nonablative procedure needs at least 80 hours of documented education and experience covering topics such as laser physics and safety, skin typing, complications and CPR.

Each patient must receive written information with the name and licensing information of the supervisor and of the person performing the procedure, plus a phone number that is answered within 24 hours.
