What the forms collect

The contact form sends: name, email, phone if you enter one, practice specialty, and message.

The Free Patient Leak Audit form sends: name, email, phone if you enter one, clinic name, website URL, city and state, message if you enter one, and a type field that marks the message as an audit request.

Both forms include a honeypot field named company_website. If that field is filled in, the server does not email the message. The server also stores a timestamp for your IP address in a temporary file so it can limit repeat submissions. The email includes the page address you submitted from, when the browser sends one.

What we do with it

The server emails the fields to hello@getpatientnow.com using PHP mail(). This site does not write form submissions to a database. We do not sell the information you submit. A reply, if we send one, comes from the GetPatientNow mailbox.

Analytics

Google Analytics and Google Tag Manager are off. No measurement ID is set in the site config, and the page does not load those scripts. If an ID is added later, those Google tools may set cookies and receive usage data. This policy would need an update at that time.

Hosting

The site is static files on Hostinger shared hosting, behind the Hostinger CDN. The host receives ordinary request logs, including IP address, browser, and the URL requested, in order to serve the site.
